When the monsoon arrives in Alappuzha, water seems to find its way everywhere. Canals rise, drains fill up, and puddles linger in corners long after the rain has stopped.
And then there are the places you may barely notice: a flowerpot left in the yard, an old tyre by the wall, a bucket collecting rainwater, a gutter that has not drained properly. To us, they may look harmless. To a mosquito looking for somewhere to lay eggs, they are exactly what it needs.
That was the problem residents of Vadackal were living with. When researchers surveyed the Alappuzha ward in 2016, they found mosquito breeding sites in 64.2 out of every 100 homes they checked.
Think about that for a moment. On a street with ten homes, breeding sites could be present in six of them. The threat of dengue was taking shape in the most familiar of places, right outside people’s doors.
Two years later, that number had fallen from 64.2 to just 8.6.
What changed? Residents had begun looking for breeding sites themselves, checking homes, talking to neighbours and acting on the places where water collected.
The effort was part of a study led by researchers from the ICMR-National Institute of Virology’s Kerala unit and TD Medical College, Alappuzha, conducted between November 2016 and October 2018. It asked a specific question: could residents themselves, organised properly, do what routine vector control drives had struggled to achieve in a district that sees dengue, chikungunya, and leptospirosis return with almost seasonal regularity?
Turning residents into their own inspectors
Before asking residents to change anything, the researchers first tried to understand what happened inside these neighbourhoods during mosquito season. Where was water collecting? What did families already know about dengue? Which prevention efforts were actually reaching homes, and which were being missed?
What emerged was a structure with real teeth. Each ward got a steering committee overseeing smaller cluster committees, drawing on the same instinct for grassroots organising that shaped Kudumbashree, Kerala’s women-led self-help network, which itself traces part of its origins to Alappuzha municipality.
These committees put up posters, walked door to door, handed out pamphlets, and ran a WhatsApp group that kept the whole effort coordinated in real time. Checking for stagnant water where mosquitoes breed stopped being an inspector’s occasional visit and became something neighbours did for each other, week after week.
When neighbours began spotting the problem themselves
Residents also noticed something the data would later confirm: uncollected garbage was trapping water and creating breeding pools of its own.
So cluster committees began clearing waste themselves, an approach that fit naturally with the town’s already well-known decentralised waste management model, built on the same principle now shaping how India handles unsegregated kitchen waste more widely.
And over time, the numbers began to reflect what residents were seeing on the ground. In Vadackal, the share of homes with mosquito breeding sites fell from 64.2% to 8.6%. In Pazhaveedu, it dropped from 16.7% to 6%.

Other indicators used by the researchers moved in the same direction.
In Pazhaveedu, the container index, which measures the share of water-holding containers where mosquito larvae or pupae are found, fell from 24.8 to 12.1. The Breteau index, which counts positive breeding containers for every 100 houses inspected, dropped from 21.3 to 7.3.
In Vadackal, the container index fell from 37.7 to 18.1, while the Breteau index dropped from 47.7 to 8.6.
Together, the numbers pointed towards a clear shift: far fewer homes and containers were harbouring mosquito breeding sites by the end of the intervention.
The researchers concluded that community engagement deserved a place at the centre of public health policy, not the edges. That argument has only grown sharper since. Kerala’s health department has repeatedly issued monsoon dengue alerts across districts, Alappuzha among them, and the state logged more than 8,000 dengue cases by mid-2024 alone.
A large part of the solution began with residents noticing the things around them: the bucket filling up in the yard, the clogged gutter, the discarded tyre holding rainwater. Then they helped their neighbours notice them too.
For a disease whose breeding grounds can sit only a few steps from the front door, that kind of neighbourhood vigilance can make a measurable difference before the next outbreak begins.
