Joe Manganiello Credits Ex-Wife Sofia Vergara For A 'Brutal But Lifesaving Decision' That Saved Him

Joe Manganiello has finally opened up about the terrifying health crisis that nearly killed him and the life-or-death decision his ex-wife, Sofia Vergara, made while he was unconscious. In his memoir Bloodlines, released October 13, 2026, the True Blood alum, 49, revealed how Sofia’s tough call made his emergency surgery a success after doctors found his large intestine was severely infected.

Manganiello says the operation left him with an ostomy bag and an uncertain future. But without his ex-wife’s intervention, he believes he might not have survived at all.

Sofia Vergara made a life-or-death decision for Joe Manganiello when he was unconscious

Joe finally opened up about one of the darkest chapters of his life, along with never-before-known details about his years-long health struggle. The actor revealed in his memoir that his health began to decline after he married Sofia Vergara in 2015. He used to feel severe abdominal pain, ulcers, fevers, rashes, headaches, and constant fatigue. Eventually, it triggered years of medical uncertainty.

Joe had to undergo emergency surgery in 2016 and spent the next few years looking for answers about his condition. The Magic Mike actor recounts how doctors contacted Vergara while he was undergoing surgery. As his next of kin, she had to decide whether to approve a procedure that would permanently change his body. He wrote:

“While I was under, they called Sofia, who, as my next of kin, had to choose what she thought I would want the doctors to do. She made the brutal but lifesaving decision to allow my entire large intestine to be removed and for a hole to be cut into my abdomen to divert my small intestine.”

Manganiello recalled that Vergara was there, waiting for him when he woke up after the ten-hour procedure. She explained that the operation had saved his life but left him with an ostomy bag for an indefinite period, possibly forever. He revealed:

“I reached down, felt it there on my stomach, and exhaled shallowly. Through my drug-induced semi-euphoria, I understood I had cheated death, but underneath that crept the fear that the life I had worked so long and hard to make for myself was over.”

During an October 12, 2026, appearance on CBS Mornings, Manganiello also acknowledged Vergara’s role in the decision. Speaking to host Gayle King, he explained that doctors had to give her the available options while he was unconscious. Joe said:

“She chose the right one, and it bought me time. It didn’t solve the problem. The doctors still had no idea what was causing this, how to treat it. But it bought me enough time.”

When King said Vergara deserved credit for saving his life, Manganiello responded, “Absolutely.”

Joe Manganiello reveals the devastating toll of his seven-year health battle that strained his marriage to Sofia Vergara

Manganiello’s memoir details the physical ordeal that followed the emergency operation. The actor says he had been struggling with unexplained symptoms for about a year before his hospitalization, which lasted about two months. But his broader health battle continued for seven years, and he stayed largely out of public view.

Only people in his close circle had any idea what the actor was going through. At one point, the pain was so severe that he had to rely on prescription pain medication, fentanyl patches, and marijuana to get through the days.

In the book, Joe describes becoming dangerously weak, losing muscle, and developing painful bedsores while recovering in the hospital. He also recounts the intensive care required after surgery, including medication, multiple catheters, and learning how to clean and change his ostomy bag. Manganiello wrote:

“After those first two weeks in Cedars, I’d dropped sixty pounds, but it looked like even more.”

Speaking to King, Manganiello recalled that his large intestine had become severely infected and was close to rupturing. Over time, the health crisis also put a strain on his marriage to Vergara. She had allegedly become emotionally distant during his prolonged health battle, to the point that she refused to hug him even when he “begged” for it. Manganiello wrote that his marriage had “become devoid of any emotionality whatsoever.”

The pair began dating in 2014 and exchanged vows in 2015. After Joe’s critical surgery, the couple started growing distant as Joe kept struggling with his health. They announced their separation in July 2023 and officially divorced in 2024.

Despite their marriage’s eventual end, Joe will always be grateful that Sofia made the life-saving decision when he couldn’t.
